The Senior Occupational Therapist will be responsible for the provision of a high quality Occupational Therapy service to the inpatients and will carry out clinical and educational duties as required. The Senior Occupational Therapist will develop and maintain high standards of clinical practice within their assigned area through clinical evaluation, audit opportunities and evidence based practice. The Senior Occupational Therapist will contribute to a structured process for education of senior and staff grade colleagues, undergraduates and other disciplines in collaboration with the clinical specialist occupational therapists on the service.

The role of the Clinical Nurse Specialist (Transient Ischaemic Attack Nurse) is to provide for the care and on-going management of patients with a TIA. The post holder will be required to work as a key member of the multi-disciplinary team in the hospital providing physical, psychological and emotional support to TIA patients and their families throughout their TIA work up. She / he may act as a liaison between TIA Services, Primary Care Teams, Community Care Teams and other agencies. The post holder will work as part of the multi disciplinary team to achieve the strategic objectives of the TIA Service.

The purpose of this CNS - Renal post is to: Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008 Caseload The CNS - Renal caseload will focus on the following patient groups: Patients with CKD and ESRD, Pre-Dialysis patients, conservative management of ESRD patients, patients requiring workup for kidney transplant and post-transplant follow up. The CNS will be accountable for the delivery of nursing care to patients. S/he will work in close collaboration with the MDT, both in the hospital and in the community setting, senior nursing, medical (including General Practitioners), pharmacy, technical and administrative staff, nursing directorate staff, nursing staff (all grades), HCA’s, human resources staff, patients, relatives, visitors, peer group, corporate facilities staff, and external agencies. The post holder in collaboration with MDT colleagues will be expected to support the development and delivery of a comprehensive service including education and timely intervention in the Pre-Dialysis and advanced CKD patient population. The role will encompass: • Delivery of a defined education programmes. • Monitoring and effectively managing the complications and symptoms associated with CKD. • Managing patients that meet the eGFR criteria to be referred to Beaumont for transplant evaluation and carrying out the required preparation/ work up. • Management of and education of the post-transplant patient. • Providing emotional and psychosocial support to patients and their families/carers. • The management of the patient population within a well-established clinic, including the prescribing of approved drugs as a nurse prescriber. • Support service development initiatives as required. On-going follow-up of this patient population will be integral to the role. This will take place in an out-patient setting and by phone. It will also include on-going clinical support to the patient’s primary care team in the community.

• The ADON Clinical Site Manager is responsible for overseeing the management of safe, efficient patient care within the hospital on a 24 hour basis 7 days per week. • This role oversees the management of the hospital out of hours including evenings, nights and weekends with varied shift times. The post involves internal rotation from days to nights. • The post holder may be required to rotate into a Directorate ADON post if and as required in line with service need. • The ADON Clinical Site Manager will work in close partnership with the Directorate ADON’s ensuring seamless leadership and management of nursing and HCA resources on a continual basis. • The ADON Clinical Site Manager may be responsible for patient flow management out of core hours including evenings, weekends and is responsible for patient flow management at night. • The ADON Clinical Site Manager will ensure that the nursing/HCA workforce is supported and deployed to meet the needs of patient care and the service on a day to day basis • The ADON in Clinical Site Management plays a vital role in contributing to the Nursing service plan, Directorate Service Plans and Corporate Strategy As part of the out of hour’s role the ADON will also have responsibility for Women’s and Children’s Services, including maternity services
